I am looking for a 360/multi-rater tool to use with a small business client. It is likely that only one individual will participate in the review with a mix of staff, vendors, and a few trusted clients as respondents. My firm would provide the follow-up, coaching and administration - I am just looking for an off the shelf assessment that makes sense for small scale use.

Check out RealTimePerformance. I can't recommend them highly enough. You can use their generic surveys, or customize. Besides results, there is an option for individual ongoing Personal Development Plans. Very reasonably priced. Sorry I didn't see your post sooner.
