Hi folks,
I've been tasked with developing an instructor evaluation and performance program for my training group. I'm looking for an instructor evaluation form that the Training Manager or his designated rep can use to evaluate an instructor's training/teaching/presentations skills.
I was thinking about going the next step and fashioning a "master training specialist" qualification program after the US Navy MTS program but I'm having trouble finding guidance on all the appropriate navy sites.
Any help on these would be appreciated.

Here's a self-assessment for instructors that you could perhaps adapt for your needs...

Instructor Competency Self-Assessment
This self-assessment is based on Instructor Competencies and Performance Indicators for the Improvement of Adult Education, sponsored by the U.S. Department of Education Division of Adult Education and Literacy. If you see yourself staying in the instructor role and growing in the field of training and development, try this self-assessment to see where you are in your development as an instructor and what areas you may want to work on improving or building.

For each item, rate yourself using the following scale:
4 = Completely Agree
3 = Agree
2 = Disagree
1 = Completely Disagree

1. I develop and maintain a knowledge base in adult learning and development.
2. I develop and maintain an in-depth knowledge base in my content area and in other relevant areas.
3. I know and I am sensitive to demands and responsibilities of adults as workers, family members, citizens and community members.
4. I know how technological systems work and how to apply that knowledge to instructional and administrative functions.
5. I know how to access information about my organization, community resources and issues, and relevant law/regulations.
6. I assess my own need for professional growth, and I develop and monitor my professional development plan.
7. I engage in a variety of professional development activities and incorporate new skills and knowledge into the learning environment to enhance the quality of my instruction.
8. I know how to plan/develop instructional strategies and materials that meet the learning goals of the student and the objectives of the program.

10. I create a physical and interpersonal climate that is conducive to learning by drawing on adult learning theory.
11. I select and apply an appropriate variety of teaching strategies.
12. I model communication, negotiation, decision-making and problem-solving skills for learners.
13. I employ individual, group, and team learning.
14. I sequence lessons logically according to the instructional objective/goal, and I pace the lessons appropriately.
15. I am sensitive to and I accommodate diverse learning styles, abilities, cultures, and experiences.
16. I provide frequent and varied opportunities for students to apply their learning.
17. I monitor and adjust teaching strategies based upon student needs and performance.
18. I effectively integrate current and appropriate media and technology as a tool for instruction.
19. I select and use a variety of resources for the learning environment.
20. I work with learners to identify their needs, strengths, goals and advise or refer them to appropriate programs and levels of instruction.
21. I use results of assessment data on a regular basis to validate and/or modify my teaching strategies.
22. I monitor learning beyond simple recall of information using a variety of strategies.
23. I structure and facilitate ways for learners to assess, develop and apply their own learning.
24. I make suggestions and/or collaborate to modify programs and develop program alternatives.
25. I share information with learners and colleagues about additional learning resources, educational opportunities, and options for accessing support services.

80-100
Wow - you are well on your way to being on top of your career as an instructor and facilitator of learning. Keep it up! Be sure to share your experiences and best practices with your peers!

60 - 79
You’re obviously aware of the types of things that you need to be a benefit to your learners and your organization. Keep working on applying that knowledge and building your skills. Sometimes, just trying a new teaching strategy, experimenting with a new idea, brainstorming with other instructors, etc. will give you new energy and help boost your continued growth and improvement as an instructor.

59 and below
You may need to be more proactive in exploring and taking advantage of self-development opportunities - workshops, books, professional organizations, peer discussions, etc. - to stay on top of your game. Review the specific items you marked as 1s and 2s to help you identify possible areas for development.

I work for the NC Justice Academy where I train Criminal Justice Offiers to be trainers. Successful completion of the program includes presenting a block of instruction that they have created. I'll be happy to forward you the forms we use to evaluate that performance. They are pretty detailed and include descriptions of performance a the various levels (above, at and below expectations). I recommend reviewing the IBSTPI (International Board of Standards for Training, Performance, and Instruction) Instructor Competencies.

I used these competencies to put together an evaluative tool for instructors in my organization, selecting those areas most relevant to the design and content of the courses.
