We know we want to begin an elearning program and would like a basic boilerplate elearning needs assessment to help us determine what options would best fit our needs.

There's no such thing as an e-learning needs assessment. You do a needs assessment first. Then you decide what training delivery method. E-learning may not be right for what you need. You are approaching this backwards.
As I understand your question...your organization has determined that it wants to explore an elearning option to compliment existing delivery methods. What you would like to do is make a first pass at getting a sense for what's both possible and appropriate for the organization.
