We use Hot Lava Software's authoring tools. They may not work for everyone, but they work for us! You can find them at www.hotlavasoftware.com.

I agree with Cathy that you should check out the Tech Smith website. I use Snag It and Camtasia and they do everything you describe for a very reasonable price.
As for authoring, what LMS are you using? It's important to make sure your authoring tool is friendly with your LMS before writing any checks.

Well, there is Lectora from Trivantis, they also have Course Mill as their Learning system. There is Captivate, but I have no real experience with it. The other I know of is CourseWorks. A lot of purchaced programs seem to use this so I could not vouch for the user friendliness.

Getting ALL of the features you mentioned is difficult to find unless you are willing to spend a huge amount.
