We use Hot Lava Software's Learning Mobile Author (LMA) and Mobile Device Tracking System (MDTS). You don't have to use mobile devices for this software, although it is a huge PLUS. You can access the testing and the real time results online via a personalized hdwap server. You can track testing in real time, don't have to have an LMS, you will have free live support and help with the set-up and it is very user friendly. And the servers are 120% secure! I have been using Quia and it does everything you need and more. I just created a quiz that randomly pulls specified categories or numbers of questions from a question bank. It also has good options for surveys and activities.
I am currently on the free 30 day trial but will not hesitate to pay the $100 per year for it once my trial expires.

There are numerous systems on the market that could do the job.

You can do a google search for 'online quiz lms' for a bunch of results.

I believe Moodle (www.moodle.org I think) -- which is free -- has quiz capabilites, although it is much more, and may be overkill.

Our Atrixware Weblearning System lets you author and deliver quizzes online, is offered as a hosted solution, or as a licence you can put on your own server, and it may be a fit as well. You can try it free to see if it is a fit.
