In my past job, we used Metrics That Matter by Knowledge Advisors and I have nothing but GREAT things to say about it! It will help you cover levels 1-4 (and 5 - ROI), and they recently launched a new add-on called Active Authoring that allows you to create custom quizzes, assessments, etc. similar and better than what you could do with Survey Monkey, Zoomerang, and QuestionMark Perception.

The good things with this tool are that
1. You can either open the assessment to a large audience or limit access to any specif group by setting up password.

2. Tracking
You can tracking the assessment results together together with assessment takers's data like name and email adress.
There are two two ways of tracking.
by mail reporting and
by database reporting.

Sounds like there is a lot of good vendors that fulfill this need. In the past I have used Survey Monkey for online surveys and been very happy. Survey Monkey has good filtering options when looking at results.

I recently started using Quiaand will probably never go back to Survey Monkey. With Quia you can make Surveys, assesments, and a variety of online games. I especially like the assesment options. I highly reccomend it.
