Jeff -- can your software be housed internally? Survey Monkey and the like will not work for me due to firewall and data privacy concerns.

Two options:

Software resides on your PC with projects processed via a web hosting site (external server). This is by far the most often selected option.

or

Same as above but the processing is done on your server.

The second option would run a bit more.
