I recently created an eLearning module using Articulate and linked to Survey Monkey at the end of the module for the final "test" activity. You can structure the questions to simulate a test and provide feedback and depending on how you structure the question, it can be self grading.

We had open-response questions which provided insights from our Users. In the end this approach was a successful solution to problems we were having with Articulate's inconsistency in email responses. Not to say that we didn't encounter some issues with Survey Monkey, as well.

Thanks for posting this! Survey Monkey is a great tool for all kinds of data collection if you are creative.
