If you provide an email address or contact information, I (or someone) could forward you one. As you haven't, a quick search turned up this one: http://ollie.dcccd.edu/mgmt1374/book_contents/4directing/trng/eval.htmIt's not the greatest, but it's something. Do you want a quick generic one to pass out, or are you looking to tailor one specifically to your training?

Chacha To help emphasise the need for trainees to be participative it can be help to add a question asking quote: How did you contribute to the session?

An interesting idea, FT, but participant contribution, while it should be a focal point of designing learning, is often, sadly, not designed into the training, nor is it well-facilitated. I was recently in a workshop where participants were encouraged to ask questions, but, frustratingly, only the facilitator answered them. She never redirected questions to the rest of the group.
It might be more meaningful to ask participants *before* training begins - How WILL you contribute to the session. That may also encourage facilitators to remember to stop playing "sage on stage" and start letting the focus be on the learners.
