I see the request for suggestions on how to do Level 1 evaluation online... and although folks seem to be very interested in discussing the value and use of Level 1, no one really answers this question.

We are looking to move our paper-based Level 1 to an online form that will allow us to generate reports based on the data (such as displaying the data in an EXCEL spreadsheet or other report).

So far we are considering using InfoPath - but it doesn't seem like the best solution.

What suggestions do you have? I appreciate your help.

I believe the reason you don't get responses for how to do it is that there is no reason to do it. Level one data is not meaningful as historic data. That's not its purpose. And meaningful level one data cannot be tracked that easily because it is the free-form feedback to open ended questions that hold the most value. Circled numbers held in databases hold little value.

Above all else -- you will greatly reduce the number of respondents if you move your immediate paper responses to online.

To me, this type of requests from businesses just demonstrates to me a complete misunderstanding of what level one feedback is.

It's cute you took the time to respond as you did considering the content of my note.

Hopefully others will not jump to conclusions about how we use this information and will kindly offer suggestions as to what tools they use to manage collecting and reviewing it.

BTW, we anticipate asking students to complete the online feedback the last few minutes of class - just as we have with paper versions. Hopefully we'll not see a large reduction in responses.

Do you have SharePoint? SharePoint has its own survey mechanism already included out of the box. It has all the features you would probably need, including branching. The survey data can be exported to Excel. Very simple. Easy to create. Easy to complete. And free if you already have SharePoint.

Not real familiar with Infopath but it seems like overkill. We use Infopath for online forms that require routing/approval, etc. More "programming" involved in that.

If you don't have SharePoint, SurveyMonkey is another option depending on volume. You can also export SurveyMonkey to Excel.

Thanks Amy! Your response is very helpful. We do have SharePoint but wondered about logistics with using it for this purpose. Based on what you've said I think we'll take another look.

I'm also hoping to have another option or two to consider...
