When you say that you are looking for a survey tool to evaluate training effectiveness, what are you trying to measure: smile sheets (level 1) or your overall training efforts (levels 1 - 4 or 5 depending on whether you are following the Kirkpatrick's or Phillip's evaluation model)? What do you mean by tools to develop and manage the survey? Depending on your needs, QuestionMark may also be an option.

I am on Lotus Notes and interested in looking at the evaluation databases.

I am also looking into web-based survey and assessment tools - I would like one software which can handle both surveys and quizzes. Checking into QuizRocket (from learningware.com) right now, for which I got one recommendation.
I did use Zoomerang in the past and found it was very easy to create surveys , but the reporting capabilities were not great (may have improved over past several years).
