My company uses NetReflector's Zoomerang tool. It has a lot of great functionality, althought it can be costly if you anticipate large numbers of responses.

Carol, I know you posted this a long time ago, but my company uses Lotus Notes as well and I would be interested in the e-survey(s) to evaluate training effectiveness.
