What is the most important thing you learned and why?
What would you most like to change and why?
What did you like most and why?
(yes - that's it)
Level 2... depends ENTIRELY on the course and what you're assessing. Depends on the objectives and the expected level of proficiency. Measuring learning can happen in many ways - does not have to be a test or what you might be thinking of as a formal evaluation.
