JUst what it is worth - just completed an internal leadership development program focused on leadership accountability...trying to instill key principles of taking charge of your organizations/teams and ridding the organization of "victim" behavior. We based the program on the principles outlined in the book "accountability...a noun or a verb?" It went really well and got strong responses from the trainees...FYI in case you have a similar need. You can find this book on amazon.com

sure - we had around 200 folks from the US and EU who were assigned key project leadership roles...these folks would ultimately be the sole accountable person for the success of these projects. As part of the initial orientation program for these new roles, we gave each leader the book as a pre-read. At the first face-to-face session, we did the nuts-n-bolts stuff about the new role and then spent quite a bit of time talking about behaviors that will drive success. With this, we put them into role play simulations around the accountability build blocks outlined in the book "Accountability...a noun or a verb?" For example, around the "can't be a victim" building block, we placed them in a scenario where a member of their team was suffering through "victim" mentality and made them come up with coaching plays for dealing with this, etc. The book itself is very simple and straightforward and each of the 7 (I think that is the right number) building blocks fit very well into scenarios/case studies for this type of development.
